Despite improvements in surgical techniques, neuroimaging, radiation therapy and new chemotherapeutic agents, high-grade gliomas are notoriously difficult to

Multiple subtypes have been identified which may alter response to treatment and prognosis. High-grade gliomas are grade III or grade IV tumors. Grade III gliomas include anaplastic astrocytomas and anaplastic oligodendrogliomas. Grade IV gliomas are called glioblastomas. High-grade gliomas grow rapidly and can easily spread throughout the brain. These are the most aggressive types of glioma and are life-threatening. These results substantiate the clinical significance of considering diffuse midline glioma, H3 K27M-mutant, as a distinct entity corresponding to WHO grade IV, carrying a universally fatal prognosis.

Diffuse high-grade gliomas with H3 K27M mutations carry a dismal prognosis independent of tumor location High-grade gliomas are highly vascular tumors and have a tendency to infiltrate diffusely. They have extensive areas of necrosis and hypoxia.

Grade 1 glioma (low-grade glioma): Grade 1 gliomas are usually occur in children and teenagers. They are the most slow-growing (low-grade) form of glioma brain tumour and carry the longest prognosis. The most common form of low-grade glioma is a pilocytic astrocytoma , which rarely progresses to a higher grade and can sometimes be completely removed by surgery .

High-grade gliomas are classified by their location and by the appearance of the tumor under microscopic examination. High grade gliomas arise from supporting glial cells in the brain. The predominant cell type determines the pathological classification.
